CagriSema (Cagrilintide + Semaglutide)
CagriSema is Novo Nordisk's investigational fixed-dose combination pairing cagrilintide, a long-acting amylin analog, with semaglutide, a GLP-1 receptor agonist. It is being studied in the company's REDEFINE clinical trial program for obesity and in a separate program for type 2 diabetes, but it is not an approved medication in its own right.
WHAT IT IS
CagriSema combines two distinct peptide classes in a single, fixed-dose product: cagrilintide, a long-acting analog of the hormone amylin, and semaglutide, a GLP-1 receptor agonist already familiar from approved products.
This is a Novo Nordisk investigational development program, not a generic or compounded blend. The company has studied this specific combination, at specific co-formulated doses, in its own controlled clinical trials under the REDEFINE name.
FDA status: CagriSema, as a combination product, is not FDA-approved. Semaglutide alone is FDA-approved under several brand names, including Ozempic and Rybelsus for type 2 diabetes and Wegovy for chronic weight management, but that approval applies to semaglutide alone, not to the cagrilintide-semaglutide combination.
Material marketed online under a generic cagrilintide-semaglutide blend name is not the same thing as the specific, proprietary formulation studied in Novo Nordisk's clinical trial program, and no equivalence between the two should be assumed.
HOW IT IS THOUGHT TO WORK
Semaglutide activates the GLP-1 receptor, a pathway involved in insulin secretion, appetite regulation and gastric emptying, and is the same general mechanism behind already-approved GLP-1 medications.
Cagrilintide is designed to mimic amylin, a hormone co-secreted with insulin that is involved in satiety signaling and slowing gastric emptying through a separate receptor pathway from GLP-1.
The rationale behind combining the two is that acting on both the amylin and GLP-1 pathways at once may produce a larger effect on appetite and body weight than targeting either pathway alone, which is the central hypothesis the REDEFINE program has been designed to test.
ESTABLISHED USES VS EMERGING RESEARCH VS MARKETING CLAIMS
It is useful to separate what is established, what is being researched, and what is marketed:
- Established: semaglutide alone is FDA-approved for type 2 diabetes and, at certain doses, for chronic weight management, under specific brand names and formulations.
- Emerging research: CagriSema, the fixed-dose cagrilintide-semaglutide combination, has been studied in Novo Nordisk's large, randomized, controlled REDEFINE trials in obesity, with a separate research program in type 2 diabetes, but has not completed the full regulatory approval process as a combination product.
- Marketing claims: products sold online as a cagrilintide-semaglutide blend are not the FDA-reviewed CagriSema formulation studied in these trials, and any implication that such products carry the same evidence base is misleading.
WHAT CURRENT EVIDENCE SUGGESTS
Novo Nordisk has run large, randomized, controlled phase 3 trials under the REDEFINE name studying CagriSema in adults with obesity or overweight, with additional trials examining its use in type 2 diabetes.
Published and publicly reported trial results have generally described reductions in body weight with CagriSema that researchers have characterized as greater than those typically reported for semaglutide alone in comparable trial populations, though exact figures vary by trial and should be reviewed directly from the primary sources rather than summarized claims.
As with other GLP-1 and amylin-pathway medications, gastrointestinal side effects such as nausea have been reported in these trials. The combination has not yet completed the regulatory review process, and its long-term safety and effectiveness profile as an approved product has not yet been fully established.
A studied formulation, not a generic blend
THE RESEARCHED COMBINATION AND A GENERIC ONLINE BLEND ARE NOT THE SAME THING
CagriSema refers to a specific, proprietary, co-formulated combination of cagrilintide and semaglutide developed and studied by Novo Nordisk in its own clinical trial program.
Products sold online under a generic cagrilintide-semaglutide blend name have not gone through that same trial program, and their formulation, dosing and manufacturing are not verified against it.
No evidence generated in the REDEFINE trials should be assumed to transfer to unregulated products marketed under a similar-sounding blend description.
