Dulaglutide
A long-acting glucagon-like peptide-1 (GLP-1) receptor agonist marketed as Trulicity, given by weekly injection and approved for the management of type 2 diabetes in adults and certain adolescents, and for reducing the risk of major cardiovascular events in adults with type 2 diabetes who have established cardiovascular disease or multiple cardiovascular risk factors.
WHAT IT IS
Dulaglutide is a synthetic peptide fused to a modified immunoglobulin fragment, engineered so it mimics the body's own GLP-1 hormone while resisting rapid breakdown, allowing once-weekly dosing.
It is not an experimental or research-only compound. It is an FDA-approved prescription medicine with a defined manufacturing standard, a detailed prescribing label and years of post-marketing surveillance.
FDA status: Approved for glycemic control in type 2 diabetes in adults and certain adolescents, and for reducing major adverse cardiovascular events in adults with type 2 diabetes who have established cardiovascular disease or multiple risk factors; not approved as a general weight-loss medication.
HOW IT WORKS AND WHAT IT IS APPROVED FOR
Its FDA-approved indications, based on human clinical trials, include:
- Improving glycemic control as an adjunct to diet and exercise in adults with type 2 diabetes
- Improving glycemic control in pediatric patients 10 years and older with type 2 diabetes
- Reducing the risk of major adverse cardiovascular events, including stroke, heart attack and cardiovascular death, in adults with type 2 diabetes who have established cardiovascular disease or multiple cardiovascular risk factors
- Acting on GLP-1 receptors to increase glucose-dependent insulin secretion, slow gastric emptying and reduce glucagon secretion
THE CARDIOVASCULAR OUTCOMES TRIAL
The cardiovascular risk-reduction indication comes largely from REWIND, a large, multi-year, randomized, placebo-controlled trial that enrolled adults with type 2 diabetes, most of whom had cardiovascular risk factors rather than existing cardiovascular disease.
REWIND reported a reduction in the composite outcome of major adverse cardiovascular events among participants on dulaglutide compared with placebo, which is the human clinical evidence underlying the cardiovascular indication.
This is a population-level trial result describing risk reduction across a large group over years, not a guarantee for any individual patient, and it does not extend to people without type 2 diabetes.
WHAT IT IS NOT APPROVED FOR
- General weight loss or weight management in people without type 2 diabetes
- Treatment of type 1 diabetes or diabetic ketoacidosis
- Use in patients with a personal or family history of medullary thyroid carcinoma (MTC) or Multiple Endocrine Neoplasia syndrome type 2 (MEN 2)
- Use as a substitute for insulin in patients who require it
WHAT THE EVIDENCE CURRENTLY SUGGESTS
For its approved indications, dulaglutide has substantial human clinical trial evidence behind it, including large randomized controlled trials reviewed by the FDA for both glycemic control and cardiovascular risk reduction.
This places it in a different evidentiary category than research peptides discussed elsewhere on this site, which typically rely on cell and animal data rather than large approved human trials.
The label also carries a boxed warning, the FDA's most serious warning category, regarding thyroid C-cell tumors observed in rodent studies, with uncertain but unruled-out relevance to humans.
Any use of dulaglutide, on-label or otherwise, belongs under the supervision of a prescribing physician who can weigh personal and family medical history, monitor labs and manage side effects.
Rodent data and boxed warnings, read carefully
AN APPROVED DRUG CAN STILL CARRY SERIOUS WARNINGS
Dulaglutide's label includes a boxed warning about thyroid C-cell tumors seen in rats and mice during preclinical testing, and it is contraindicated in people with a personal or family history of medullary thyroid carcinoma or MEN 2.
It is unknown whether dulaglutide causes these tumors in humans, and the boxed warning reflects an abundance of caution based on animal findings rather than confirmed human cases.
An FDA approval does not mean a drug is free of serious risk. It means regulators judged, for defined indications and populations, that the documented benefits outweigh the documented risks under physician supervision.

What we know
Dulaglutide is FDA-approved for glycemic control in type 2 diabetes in adults and in children 10 and older, and for reducing major adverse cardiovascular event risk in adults with type 2 diabetes who have established cardiovascular disease or multiple risk factors, based on large randomized human trials including REWIND.
What remains uncertain
Its long-term relevance to thyroid C-cell tumor risk in humans remains unresolved, and it is not studied or approved for general weight loss, for people without type 2 diabetes, or for any indication in patients with a personal or family history of MTC or MEN 2.
