WHAT IS TIRZEPATIDE?

Tirzepatide is a synthetic peptide-based medication that activates two important metabolic signaling systems: the glucose-dependent insulinotropic polypeptide receptor, commonly called GIP, and the glucagon-like peptide-1 receptor, commonly called GLP-1.

These pathways are involved in blood glucose regulation, appetite, digestion and energy balance.

Tirzepatide is marketed under the brand name Mounjaro for type 2 diabetes and Zepbound for chronic weight management in appropriate adults. Zepbound is also FDA approved for moderate-to-severe obstructive sleep apnea in adults with obesity.

WHY RESEARCHERS ARE INTERESTED

Tirzepatide has attracted significant scientific attention because it targets both GIP and GLP-1 receptors rather than GLP-1 alone.

Large randomized human clinical trials have demonstrated substantial reductions in body weight along with improvements in glucose control and multiple cardiometabolic measures.

HOW IT WORKS

Tirzepatide mimics signaling from two naturally occurring gut hormones involved in metabolism.

Its actions can:

  • Influence appetite and satiety signaling
  • Reduce overall food intake

WHAT THE RESEARCH SHOWS

Tirzepatide has been evaluated in large randomized clinical trials involving thousands of participants.

In the landmark SURMOUNT-1 obesity trial, adults with obesity or overweight achieved substantial average reductions in body weight over 72 weeks. Mean weight reduction was approximately 15% with 5 mg, 19.5% with 10 mg and 20.9% with 15 mg, compared with approximately 3.1% with placebo.

Research has also shown that continued treatment is important for maintaining weight reduction. When tirzepatide was withdrawn in the SURMOUNT-4 trial, many participants regained a meaningful portion of previously lost weight.

BODY COMPOSITION: FAT LOSS AND LEAN MASS

Scale weight does not tell the entire story.

A body-composition substudy of SURMOUNT-1 found that tirzepatide reduced both fat mass and lean mass, with the majority of lost weight coming from fat mass.

This is an important consideration for anyone undergoing substantial weight loss.

Maintaining adequate protein intake, resistance training and appropriate nutrition can be important components of a broader strategy designed to support muscle and functional strength during weight reduction.

Tirzepatide does not specifically “attack muscle.” Loss of some lean tissue can occur during substantial weight loss regardless of the method used.

Tirzepatide is a prescription medication.

Common adverse effects are primarily gastrointestinal and can include nausea, diarrhea, vomiting, constipation, abdominal discomfort and decreased appetite.

FDA prescribing information contains a boxed warning regarding thyroid C-cell tumors observed in rats. It is unknown whether tirzepatide causes medullary thyroid carcinoma in humans.

Tirzepatide is contraindicated in individuals with a personal or family history of medullary thyroid carcinoma or Multiple Endocrine Neoplasia syndrome type 2.
